TERMS AND CONDITIONS FOR CHILDCARE SERVICES

These terms and conditions ("Terms") apply to the childcare services provided by [Name of Childcare Service Provider] ("Provider") to the parent(s) or legal guardian(s) of the child(ren) who will be attending the Provider's childcare facility ("Parents").

  1. Registration and Enrollment:

a. Parents must complete and submit the Provider's enrollment form, providing all required information about the child(ren), including name, age, medical conditions, allergies, and emergency contact information.

b. The Provider reserves the right to refuse enrollment if the child(ren) has any medical condition that requires medical treatment beyond the scope of the Provider's staff training.

c. The Provider reserves the right to conduct background checks on Parents and/or guardians, as well as anyone who will be picking up the child(ren) from the Provider's facility.

  1. Hours of Operation:

a. The Provider's facility is open [7:30 to 4:30], Monday through Friday, except on public holidays.

b. Parents who require childcare services outside of the Provider's regular operating hours may be charged an additional fee.

  1. Fees:

a. Parents must pay all fees in advance of the childcare services, either on a weekly, bi-weekly, or monthly basis.

b. The Provider reserves the right to change fees at any time, with notice to the Parents.

c. If the Parents fail to pay the fees when due, the Provider may terminate the childcare services.

  1. Cancellation:

a. If the Parents need to cancel the childcare services, they must provide at least [insert notice period here] days' notice in writing to the Provider.

b. The Provider reserves the right to charge a cancellation fee if the Parents fail to provide the required notice.

  1. Termination:

a. Either the Provider or the Parents may terminate the childcare services upon 3 days' written notice to the other party.

b. The Provider may terminate the childcare services immediately if the Parents breach any of these Terms or fail to pay the fees when due.

  1. Health and Safety:

a. The Provider will take all reasonable steps to ensure the health and safety of the child(ren) in its care.

b. Parents must provide all necessary medical information about the child(ren), including any allergies or medical conditions, and must notify the Provider in writing of any changes to this information.

c. Parents must ensure that their child(ren) are free from any contagious illness or disease before attending the Provider's facility.

  1. Liability:

a. The Provider is not responsible for any loss or damage to personal property brought onto its premises by Parents or the child(ren).

b. The Provider is not responsible for any injury or illness suffered by the child(ren) while in its care, except where such injury or illness results from the Provider's negligence.

c. The Provider is not liable for any indirect, incidental, or consequential damages arising from the provision of childcare services.

  1. Privacy:

a. The Provider will keep all personal information about the child(ren) and their families confidential, except as required by law.

b. The Provider may use photographs or videos of the child(ren) for promotional purposes, provided that the Parents have given their consent.

  1. Governing Law:

a. These Terms shall be governed by and construed in accordance with the laws of Florida.

b. Any dispute arising from these Terms shall be resolved by arbitration in accordance with the rules of the DCF.

c. The decision of the arbitrator shall be final and binding on both parties.

By signing the enrollment form, the Parents agree to these Terms and certify that they have read and understood them.
